Family Care Teams are interdisciplinary teams that work collaboratively to best meet the needs of patients and families. We’ll be joined by leaders from NL Health Services and the Department of Health and Community Services for a discussion on the implementation of Family Care Teams throughout the province and how it will improve the way we access primary health care in NL.
